All Cosmetic Procedures must be paid for prior to treatment. We accept Cash, Personal Checks, and Mastercard / Visa / American Express.
Please bring your insurance card to your appointment, along with your referral information. You may also have your primary care physician fax your referral to our office at (312) 695-4303
Check the NMFF website to see if your insurance is one of many accepted.
The Patient Services Center works to make sure your experience with the NMFF is a good one. If you or your family has a question or concern about your stay at our hospital, please let us know. We suggest you first discuss your concerns with your nurse, the department manager and your doctor. If your concern remains unresolved, the Patient Services Center staff is available to help reach a resolution or provide more information. Patient Relations representatives are available at (312) 695-1920.
Convenient parking is available in the parking lot at the corner of Huron and St. Clair Streets. You may enter this lot from either Huron or Superior Street. Discount parking is available to patients. Please ask your physician’s office staff for details.

Physicians wishing to refer patients to Dr. Thomas for consultation or second opinion are welcomed to contact the office for an appointment (312) 695-8182. Any pertinent medical documents, photographs, or other information may be faxed (312) 695-4303 or mailed to the office for Dr. Thomas’ review. Once the patient is seen, we will promptly return a dictated letter and any other relevant information related to patient care.
